Informal Probate Forms

The North Dakota Supreme Court has assembled a guidebook, Informal Administration of an Estate, and a series of forms that may be used with it. You can access the guidebook and forms by clicking on the links below.

The forms are in portable document format. Viewing them requires the use of Adobe Acrobat Reader, a free downloadable program that can be found at the Adobe website.

You can fill in the blanks of the forms listed below by typing in the requested information on your computer keyboard. Print out each form when you complete your work. The form will clear when you close it.

If you do not want to fill out the forms on your computer, you can print out blank forms and fill them in by hand.

WARNING: Some of the forms below may ask you to provide social security numbers, credit, debit, or electronic fund transfer card numbers, or other financial account numbers. Under N.D.R.Ct. 3.1(j), if you are required to use these numbers on a form, you must mark the form “Confidential” so that the clerk will know to place it in a restricted part of the file. You must also submit a second copy of the form to the clerk with the numbers marked out so the clerk can file it in the public part of the file.
