You can fill in the blanks of the forms listed below by typing in the requested information on your computer keyboard. Print out each form when you complete your work. The form will clear when you close it.

WARNING: Some of the forms below may ask you to provide social security numbers, credit, debit, or electronic fund transfer card numbers, or other financial account numbers. Under N.D.R.Ct. 3.1(j), if you are required to use these numbers on a form, you must mark the form “Confidential” so that the clerk will know to place it in a restricted part of the file. You must also submit a second copy of the form to the clerk with the numbers marked out so the clerk can file it in the public part of the file.

NOTE: Effective July 1, 2005, N.D.C.C. 27-08.1-04 was amended to include this language: "If the defendant elects to remove the action from small claims court to district court, the district court shall award attorney's fees to a prevailing plaintiff."
